What should the role of the states be?

What should the role of states be? Should states have a role, though limited as in the case of the ACA (ObamaCare)? Or should states have no role, as in the case of Medicare? Should some get insurance in their own state, with networks of providers? Or should there be no networks and all each person be able to go to any provider in the country?
